Meet Your Home Health Care Team

Willowbrook Visiting Nurse Association puts together a team just for you based on the treatment plan prescribed by your doctor. Your team can include:

Your Physician
Your Physician prescribes the level of home care and medication necessary for your recovery or maintenance of your health. Your physician certifies that you are eligible to receive home health services, directs your plan of care and receives updates from Willowbrook Visiting Nurse Association on a regular basis.

Skilled Nurses
Willowbrook’s Skilled Nurses will visit you at home to evaluate your health care needs, monitor your condition, and determine the range of services needed to carry out your physician’s treatment plan. Nurses provide treatments and educate you and your family about your care. They will instruct you about your medications, home safety and how to manage your health care condition to support your ability to remain independent in your home.

Physical Therapists
Physical Therapists help clients who have had surgery, an injury or an illness which has affected their ability to move and participate in everyday activity and/or work. Willowbrook’s physical therapist will address your and your physician’s goals by focusing on specific exercise programs to improve strength, endurance, balance and flexibility. Your personalized program will also focus on areas such as getting in and out of bed, getting in and out of a chair and car, walking, climbing stairs, and wheelchair mobility, as well your ability to return to your favorite leisure activities or hobbies.

Occupational Therapists
Occupational Therapists teach people with physical, cognitive and/or visual problems how to regain their activities of daily living to live as independently as possible. Our therapists look at the whole person to help them reach their goals. We help clients re-learn everyday activities such as dressing, bathing, toileting and more.

Speech Therapists
Willowbrook Visiting Nurse Association’s Speech Therapists help clients who have swallowing and/or communication difficulties as a result of a stroke, surgery, brain injury or other disease processes. Speech Therapists can assess your ability to swallow, make recommendations for an appropriate diet and teach you and your caregiver to monitor safe swallowing. Speech Therapists can also assist you if you are experiencing challenges with communication such as difficulty speaking or understanding words.

Home Health Aides
If you are receiving other services from Willowbrook Visiting Nurse Association, you may be eligible for personal care services from a Home Health Aid. The Home Health Aide follows a plan of care that is written by a Registered Nurse or a Therapist based on your needs. Assistance with bathing or dressing are examples of the kind of care provided. All of our home health aide staff undergo extensive training and are certified in the State of Tennessee.
